As the largest provider of rehab & skilled nursing services in Chautauqua County, Heritage Ministries Management Company provides administrative services at three convenient locations. Each of these facilities provides skilled nursing and rehabilitative care in environments designed to promote a purposeful lifestyle.

The first rehab & skilled nursing facility was built in Gerry, New York in 1972. Heritage Village accomplished the vision of having a full continuum of care available on our Gerry, NY campus. Since that time, Heritage Village has been recognized among the best nursing facilities in New York State for several years.

Heritage Park (Jamestown, NY) and Heritage Green (Greenhurst, NY) officially became members of The Heritage Group family on January 1, 1995. Both of these facilities operated as nursing homes prior to our acquisition.

Each rehab & skilled nursing facility is licensed by the New York State Department of Health. All facilities allow Medicare, Medicaid and supplemental insurances as acceptable forms of payment.
