Connections ResidentsOctober 2008 - Residents of Heritage Green’s “Connections” dementia care program recently had the opportunity to assemble prayer bracelets for military men and women in Iraq.  The soldiers use the prayers bracelets for good luck and insert them into their helmets, boots, and pockets.  “Our Connections residents enjoy the feeling of helping others and this program allows them to accomplish this," states Deb Johnson, Connections Activities Assistant.  “They are more than happy to provide a needed emotional lift to our service men and women in Iraq.”

Connections is a multi-faceted, person-centered dementia care program designed to engage skilled nursing residents in purposeful activities.  Connections includes an in-patient day center that focuses on and validates a residents’ remaining cognitive, emotional, physical, and social abilities, rather than their losses.  In addition to being offered at Heritage Green, Connections is now also being offered at Heritage Park Rehab & Skilled Nursing (Jamestown) and Heritage Village Rehab & Skilled Nursing (Gerry).
