A skilled nursing facility is a residence that provides continuous medical service to residents requiring extensive nursing or rehabilitative care by registered nurses, based on treatment prescribed by the resident’s personal or on-site physician. At Heritage Ministries, traditional long-term care is a thing of the past. Our rehab and skilled nursing facilities are no longer solely places where individuals spend their last years, but instead, caring, supportive environments that help active adults return to their homes. Each of these facilities provides skilled nursing and rehabilitative care in environments designed to promote a purposeful lifestyle. Heritage Park , Heritage Green , and Heritage Village , all house numerous adults working toward this goal. Each rehab & skilled nursing facility is licensed by the New York State Department of Health. All facilities allow Medicare, Medicaid, and supplemental insurances as payment. | | Q: What is Adult Day Care? | | A: Adult Day Care programs assist caregivers who have responsibility for an older adult who cannot be left alone during the day. Adult Day Care provides supportive staff, a secure group setting, supervision, meals, social work services, recreation and socialization.
