Bariatric Care Program

 

Due to the dramatic rise in obesity, the demand for care of bariatric residents in rehabilitation and skilled nursing facilities has steadily increased over the years. Heritage Ministries has recognized this trend and is proud to offer the needed care, equipment, and space needs of these residents in order to properly provide that care and prevent injury.  The Heritage Bariatric Care program is the first of its kind in the Western New York area. 

We provide our bariatric residents with a team approach to care with a dedication to behavioral and life support of long term weight management success.

The Heritage Bariatric Care program features specialized equipment including wheelchairs, beds, mattresses, commodes, walkers, and geri-chairs that are all designed for the comfort of our residents.

In addition to these services, our program involves physical and occupational therapy designed to increase mobility and encourage weight-loss. Our therapists help motivate the residents to their optimal level of functioning.

Our Bariatric Care program is fully supported by our skilled nursing team including dietician, social worker, and the activity department. These individuals provide emotional and behavioral support, proper diets, and an appropriate exercise plan.

Individualized Dietary Services

The Dietary aspect is an important part of the overall Heritage Bariatric Care program. Heritage provides a basic diet plan for post gastric by-pass or lap banding residents.  The Registered Dietician, in conjunction with the referring physician and bariatric resident, formulate individual diet plans during phase I, phase II and phase III of the bariatic diet.

The diet plan includes protein supplementation as prescribed by the referring physician.  Bottled water is also provided to the bariatric resident for constant hydration.  Weight-loss is monitored weekly by the Dietician and Nurse Practitioner.  As part of the program, diet counseling is provided during the resident's stay and prior to discharge.
