Heritage Park Rehab & Skilled Nursing is home to 146 residents in need of long-term and rehabilitative care services. It is located in the heart of Jamestown, New York.

The best feature of Heritage Park is its dedicated staff. Every staff member is committed to providing each resident with care and services tailored to the resident's particular needs. Nurses, social workers, dieticians, and physical, occupational and speech therapists work together as a team to meet the emotional, physical and spiritual needs of every resident.

In addition to providing quality care, Heritage Park is also committed to giving residents an enjoyable lifestyle. Several activities are planned each month, including birthday and holiday parties, bowling, dancing, playing cards, discussion groups, and numerous community activities. Each summer "The Park" hosts its annual Resident Family Picnic complete with hotdogs, hamburgers, pony rides, clowns, face painting, a petting zoo and much, much more!
