1. Payment Requirements & Methods

Payment Timing: Payment for transportation services is required at the time of pick up.

Accepted Methods: Payment can be made by cash or check.

Check Payable To: Checks must be made payable to Heritage Ministries.

2. Service Protocol and Financial Accountability

Service Funding Prerequisite: Full payment is required at the time of pick up for the scheduled transportation to proceed. Our staff is unable to initiate the trip until the payment obligation is fulfilled.

Cancellation Fee: A fee of $30 will be assessed for the following reasons:
• Same-day/next-day cancellation (per resident/family request).
• Trips that cannot be completed due to failure to meet the payment prerequisite.

3. Transportation Rates & Quotation Protocol. All transportation costs are variable and depend on the specific details of the trip

Quotation Requirement: The final rate will be quoted at the time of the transportation request for all destinations.

Rate Factors: The quoted rate is based on the following three factors:
1. Resident location.
2. Location of the appointment.
3. Anticipated wait time (for out-of-town trips).
