Heritage offers individualized therapy that covers physical, occupational, and speech-language therapies.
Our goal-oriented programs are patient-centered and customized to provide the support you need in
consideration of abilities, age, and diagnosis. Through evidence-based interventions, adaptive techniques and
assistive devices, we help you adjust and get back to a higher quality of life.

THERAPY SERVICES AT HERITAGE

Physical therapists are movement experts who improve quality of life through prescribed exercise, hands-on care, and patient education.  PTs examine each individual, and develop a plan using treatment techniques to improve the ability to move, reduce pain, restore function, and prevent disability. Physical therapists also take preemptive measures, working with individuals to prevent the loss of mobility before it occurs through the development of targeted fitness and wellness programs.

Occupational therapy practitioners help older adults to safely do the things they want to do through the therapeutic use of everyday activities (occupations) – helping people recover from injury to regain skills, and providing supports for older adults experiencing physical and cognitive changes.  Occupation includes all the activities or tasks that a person performs each day.  For example, getting dressed, completing a hobby or activity, managing medications, or cooking a meal.

Speech Language Pathologists provide life-changing treatment, support and care for individuals who have difficulties with communication, eating, drinking and swallowing. SLPs help people who, for physical or psychological reasons, have problems speaking and communicating.

REHAB PROGRAMMING AT HERITAGE

Our programming is individualized and resident-centered to meet the goals and expectations of each individual resident served while also applying the principles of our mission – offering Hope, Dignity, and Purposeful Living. Most often, our programming goals are geared to facilitate a safe & fast transition to home, one’s senior housing community, or to an assisted living community. Short-term rehab is available following a hospitalization, or when one is having difficulty safely managing within their current living environment.  The rehab goals facilitate return to the appropriate level of independence required for each discharge destination.

Upon arrival to our skilled nursing community, our residents receive an immediate skilled nursing assessment from our experienced nurses. Within 24 hours of admission, each short-term resident receives a comprehensive PT & OT evaluation in order to immediately implement the rehab programming. Speech-language pathologists are also immediately available when needed to assist with swallow therapy, speech-language therapy, and/or for cognitive & memory care concerns.

Heritage offers outpatient therapy specializing in treatment for individuals following elective orthopedic surgeries (such as knee & hip replacement), and for those with arthritis, pain, stroke/CVA, dysphagia, Parkinson’s disease, or work-related injuries. We offer outpatient therapy specializations that include a Certified Ergonomics Specialist II, a Lymphedema Specialist, LSVT Big & LSVT Loud for individuals with Parkinson’s. Patient satisfaction is our top priority and we provide 1:1 outpatient appointments! We offer traditional interventions as well as the use of the following modalities.

  • Manual techniques including joint mobilization & myofascial techniques
  • Ultrasound and electrical stimulation
  • Moist heat and cold
  • Customized home exercise programs

Outpatient services are found at The GreenThe Green is open to the community, the employees of Heritage Ministries, and is to assist with a safe transition home following a short-term rehab stay. After a short-term rehab stay, our residents can return for outpatient therapy as soon as the day after discharging from our rehab unit to continue therapy services with the same therapists, which they have grown to know and trust. Outpatient therapy after short-term rehab assists to ensure a safe & successful return to home.

Mobile outpatient therapy is also available as a part of the CCRC services available for independent and assisted living residents on our Gerry campus. Our PT, OT, and/or SLP will go to your home, the senior apartments, or the Bergquist Memorial assisted living community to offer services which improve ability to age in place.

Heritage offers therapy specializations for short and long-term rehab and is the area’s premier therapy provider for individuals with Parkinson’s disease & Dementia care.

  • Parkinson’s Specialists. Physical therapists certified in LSVT Big and has speech language therapists certified in LSVT Loud and certified through the Parkinson’s Voice Project offering the SPEAK OUT! & LOUD Crowd! programs are available.
  • Dementia Specialists. We have multiple occupational therapists, physical therapists, and speech-language pathologists at each skilled nursing community trained through Dementia Capable Care.
  • Lymphedema Specialist. A Lymphedema specialist is available at The Park and The Green, and for outpatient occupational therapy.
