HEALTH INSURANCE | NOVA
Heritage offers group medical insurance administered by Nova Healthcare. Choose between our basic, low or high deductible plans for coverage that’s right for you and your dependents. Doctor visits, maternity care, urgent/emergency care, hospital stays, therapy, prescription drug, and other medical services are available through out nation-wide network of providers. Elect health insurance benefits through Kronos.
DENTAL INSURANCE | MET LIFE
Heritage offers its employees the opportunity to participate in a voluntary dental plan through Met Life, offering a stronger and wider network of participating providers. Orthodontia is available for dependents only under the Employee + Children and Family tiers. Elect dental insurance benefits through Kronos.
VISION INSURANCE | VSP
Through VSP Vision Insurance, you’ll enjoy more value and the lowest out-of-pocket costs along with the best care and the most comprehensive exam designed to detect eye and health conditions. You can choose a VSP doctor, a participating retail chain, or any out-of-network provider. Elect vision insurance benefits through Kronos.
HRA AND FLEX PLANS (FSA + DCA) | P&A
Heritage offers employees the opportunity to participate in various spending accounts through P&A Group. An Health Reimbursement Arrangement (HRA) is an employer funded account setup to reimburse a specific amount of medical expenses incurred under the health plan. A Health Care Flexible Spending Account (FSA) is for expenses not covered by medical, vision, and dental plans, such as dental and optical care, prescription drugs, certain over-the-counter drugs, health and dental deductibles, co-payments, etc. A Dependent Care Account (DCA) is for expenses for the care of dependents allowing an employee (and their spouse) to work or to attend school full-time. Eligible expenses include pre-school & nursery school program, day care, and after school programs. Choose to participate in flex spending benefits through Kronos.
LIFE INSURANCE | MET LIFE
Basic Life and Accidental Death and Dismemberment Insurance is offered all eligible employees through Met Life. This policy is at no cost to the employee. Name your life insurance beneficiaries through Kronos annually.
Supplemental Life and Accidental Death and Dismemberment Insurance is also available to eligible employees through Met Life. This optional benefit may be purchased for the employee, the employee’s spouse, or children.
VOLUNTARY BENEFITS | MET LIFE
Voluntary Critical Illness Insurance benefit provides a lump sum benefit payment upon diagnosis of any qualified Critical Illnesses listed under covered conditions. The benefits are paid even if medical insurance is paying 100% of the cost. Your cost is based on your age and amount of coverage you select. There will be cost adjustments as you age. You must elect coverage for yourself to cover your spouse and/or children.
Voluntary Accident Insurance benefit provides a lump sum benefit payment upon one of the covered accident conditions. There are two options available, a low plan and a high plan. The benefits are paid even if medical insurance is paying 100% of the cost. Your cost is based on your selected tier of coverage.
Voluntary Hospital Indemnity Insurance benefit provides a lump sum benefit payment upon hospital admission or confinement. There are two options available, a low plan and a high plan. The benefits are paid even if medical insurance is paying 100% of the cost. Your cost is based on your selected tier of coverage.
Voluntary Long-Term Disability Insurance benefit provides off the job coverage for accident and injury. Voluntary Long-Term Disability benefit replaces a portion of your pre-disability earnings, less any offsets from other disability programs or social security payments. This plan has a pre-existing limitation of 3/12. Please see the MetLife Plan Certificate for a full explanation of benefits.

The Short-Term Disability benefit replaces a portion of your pre-disability earnings, less any offsets from other disability plans. 401K RETIREMENT PLAN | DEFINITI
Heritage offers a 401k deferral retirement plan to all employees who are at least 19 years of age. Eligibility is effective on the first of the month following 30 days of hire. Our plan allows an employee to dedicate a percentage of their pre-tax salary to a retirement account. Name your 401k beneficiaries through Definiti annually.
IN-HOUSE PHARMACY BENEFIT
All locations and affiliates of Heritage Ministries nationwide have access to our in-house pharmacy which offers medication at reduced costs and copays (compared to national retailers). Order over-the-counter medications at reduced costs or have your doctor set up prescriptions to be filled by Heritage Pharmacy in Gerry, NY. EMPLOYEE ASSISTANCE PROGRAM
Heritage offers help to you and your family have access to professional support and guidance to make life a little easier. Our Employee Assistance Program (EAP) is available to you and provides easy-to-use services to help with the everyday challenges of life — at no additional cost to you.
PAID TIME OFF
PTO is available to full-time Heritage employees nationwide. The amount of PTO earned depends on your length of service and hours worked. Eligible employees will earn PTO time as they work, beginning on their first day of employment with Heritage Ministries. Employees must complete the thirty (30) day benefit eligibility waiting period prior to using their PTO. Employees will earn PTO on all regular hours worked, PTO time used and sick time used. There is no accrual of PTO for part-time, casual, per diem, students, and weekend only employees. Contact Human Resources or the Benefits Office with questions regarding paid time off accrual and usage.
Heritage Ministries observes the following holidays: New Year’s Day, Easter Sunday, Memorial Day, July 4th, Labor Day, Thanksgiving Day, and Christmas Day. All full time employees working 30(+) hours per week will receive holiday pay at their regular rate of pay and typical schedule for the holidays listed above.
